Minutes from March 4, 2008 Meeting

 

Call to Order:  Meeting called to order at 7:00 p.m. by Louis Masiello.  Members present: Louis Masiello, Linda Firth, Anthony Tuccelli, Joanne Housianitis, and Andrea Brochu, SBOH Secretary.   

 

Public Comments: None.

 

Old Business: 

 

1.         Health Fair – Andrea will send the list of vendors who are participating in the next weekly packet.  She will also check with Bobbi Klima regarding advertising it on cable.

 

2.         Discussion regarding late fees – Andrea will re-draft renewal letter and send in weekly packet.

 

New Business:

 

1.         Massage Therapy Licenses

SBOH Action:  Motion made by Anthony Tuccelli; seconded by Joanne Housianitis.  Unanimously voted to rescind the Requirements for Operating as a Massage Therapist dated March 16, 2001.

 

2.         Letter from Attorney Desfosses regarding 46 Kendall Lane – Robert Fraser, Interim Health Agent will set up a meeting with Attorney Desfosses at the property to go over violations.  Mr. Place spoke on behalf of his ex-wife.

 

Licenses/Building Permits:

 

1.                  Site Plan review of Subdivision: 85 Folly Mill Road

SBOH Action:  Motion made by Joanne Housianitis; seconded by Anthony Tuccelli.  Unanimously voted to table until next meeting.  

 

2.                  License Revocation: Tanning; Creative Cuts, Maryana Grigoryan

SBOH Action:  Motion made by Anthony Tuccelli; seconded by Joanne Housianitis.  Unanimously voted to send a certified letter.

 

3.                  Renewal of Sewer Waiver: 116 Rabbit Road

SBOH Action:  Motion made by Joanne Housianitis; seconded by Anthony Tuccelli.  Unanimously voted to approve pending inspection by Alexander Parker.

SBOH Action: Motion made by Linda Firth; seconded by Joanne Housianitis.  Unanimously voted to renew license.

 

5.         License Renewals: Food;  Marshview Café, Mapow

SBOH Action: Motion made by Linda Firth; seconded by Joanne Housianitis.   Unanimously voted to renew food licenses for items 5 and 6 on the agenda.

 

7.         License Renewal: Food Service/Take-Out; Salisbury Mini-Mart d/b/a Subway, Submarine Restaurant, Exit 60 Mobil on the Run, Sal’s Pizza, Salisbury Donuts LLC, Salisbury Scrivano d/b/a Dunkin Donuts, Pines Camping Area

SBOH Action:  Motion made by Linda Firth; seconded by Anthony Tuccelli.  Unanimously voted to renew food licenses for items 7 through 13 on the agenda.

 

14.       License Renewal: Prepackaged Food with Milk; Salisbury Mini-Mart, Krishana Corp. d/b/a Marte-L Convenience, Tom’s Discount, Shaheen, Inc. d/b/a Melo’s Liquor Store, Performance Nutrition, The Brazilian’s Place, Rusnik, Inc., Salisbury Discount House  Annex, Pines Camping Area

SBOH Action: Motion made by Linda Firth; seconded by Joanne Housianitis.  Unanimously voted to renew licenses for items 14 through 25 on the agenda, excluding items 15, 16 and 17.

 

15.       License Renewal: Prepackaged Food with Milk; Pushcart Caterers, Salisbury Baseball/Softball League, Lion’s Park, Salisbury Baseball/Softball League, Beach Field

SBOH Action:  Motion made by Linda Firth; seconded by Anthony Tuccelli. Unanimously voted to renew licenses for items 15 through 17 on the agenda pending inspection by the Health Officer.

 

26.       License Renewal: Ice Cream; Hodgies Too, Salisbury Dairy Queen

SBOH Action: Motion made by Linda Firth; seconded by Joanne Housianitis. Unanimously voted to renew licenses for items 26 and 27 on the agenda pending inspection by the Health Officer.

 

28.       License Renewal: Fruit/Produce; Bartlett’s Farm Stand

SBOH Action: Motion made by Linda Firth; seconded by Anthony Tuccelli. Unanimously voted to renew license pending inspection by the Health Officer.

29.       License New: Massage Hannah Reed and Renewal Eileen Modricker

SBOH Action: Motion made by Linda Firth; seconded by Joanne Housianitis.  Unanimously voted to remove items 29 and 30 on the agenda without prejudice due to changes of state regulations pursuant to a letter from the Division of Professional Licensure  dated January 2, 2008.

 

31.       License Renewal: Motel; Johnson’s Motel, Hilton Dunes Motel, Driftwood Motel

SBOH Action: Motion made by Linda Firth; seconded by Anthony Tuccelli.  Unanimously voted to renew items 31 through 33 on the agenda pending inspection by the Health Officer.

 

34.       License Renewal: Pool; Pine’s Camping Area

SBOH Action: Motion made by Linda Firth; seconded by Joanne Housianitis.   Unanimously voted to renew license pending inspection by the Health Officer.

 

35.       License New: Septic Installer; Steve LeBlanc

SBOH Action: Motion made by Linda Firth; seconded by Joanne Housianitis.   Unanimously voted to grant license pending passing Septic Installer’s exam.

 

36.       License Renewal: Septic Installer; John Divincenzo, Kevin Greaney

SBOH Action: Motion made by Linda Firth; seconded by Joanne Housianitis.   Unanimously voted to renew items 36 and 37 on the agenda pending passing Septic Installer’s exam.

 

38.       License Renewal: Septic Hauler; Stewart’s Septic Service

SBOH Action: Motion made by Linda Firth; seconded by Joanne Housianitis.   Unanimously voted to renew license.

 

Other Issues:

 

1.         State Reservation – Andrea will check with Alex to see if they have a dumping station. 

 

2.         Port A Potty – Discussion relative to contractor’s obtaining a license for Port A Potty’s on construction sites.

 

3.         Workshop – Scheduled for March 18 meeting regarding outstanding regulations.

 

4.         Status of letters regarding Well Regulations to homeowners – Andrea has not done this yet; it is on her list.

5.         Tobacco Permit Fees – We will hold a public hearing on April 1st regarding raising fees.   Andrea will check with Newburyport, Amesbury and Merrimac to see what they charge for a tobacco permit fee. 

 

6.         62 Rabbit Road – Have Alex follow-up to see if it has been pumped per pumping order.
