Call to order: Meeting called to order at 7:00 P.M. by Louis Masiello, Chairman .

Members present: Linda Firth, Joanne Housianitis and Marie Blais SBOH secretary.

1. Gourmet Pantry: Request to add equipment to store

SBOH Action: Motion by L. Firth; seconded by Joanne Housianitis, Unanimously voted to table. Request menu which requires use of equipment.

2. Heritage Park: Leach field maintenance

Bonnie Ellis, Director of Heritage Board and Todd McGrath from Millenium Engineering presented a plan of maintenance for the shared field.

SBOH Action: Motion by L. Firth; seconded by Joanne Housianitis, Unanimously voted to authorize scheduled events as proposed.

Licenses/Building Permits:

1. 65 Pike St.: Building permit: Endorse Health Officer Horace Baxter’s signature.

SBOH Action: Motion by L. Firth; seconded by Joanne Housianitis, Unanimously voted to endorse Health Officer Horace Baxter’s signature.

2. 105 Bridge Rd: Site Plan Review

SBOH Action: Motion by L. Firth; seconded by Joanne Housianitis, Unanimously voted to deny plan until water line going from building to main source is shown.

3. 75 Main St.: Building Permit: Endorse Health Officer Horace Baxter’s signature.

SBOH Action: Motion by Joanne Housianitis; seconded by L. Firth. Unanimously voted to endorse Health Officer Horace Baxter’s signature.

4. License renewal: Septic Installer: Thomas Kelly, Harold Congdon, Wayne Harrison, Greg Harrison, Brian Farmer

SBOH Action: Motion by Joanne Housianitis; seconded by L. Firth. Unanimously voted to approve licenses.

5. License new: Septic Installer: Horace Baxter, Angela Silva, Jarod Meekins, Charles Bagley, Thomas Finn

SBOH Action: Motion by L. Firth; seconded by Joanne Housianitis. Unanimously voted to grant licenses

SBOH Action: Motion by Joanne Housianitis; seconded by L. Firth. Unanimously voted to approve license.

8. License renewal: Food take-out: Subway, Lee Hunt’s Seafood

SBOH Action: Motion by L. Firth; seconded by Joanne Housianitis. Unanimously voted to renew license pending inspection by Health Officer

9. License renewal: Pre-packaged food: Richdale

SBOH Action: Motion by L. Firth; seconded by Joanne Housianitis. Unanimously voted to renew license if inspection has been completed.

10. License renewal: Fruit and Vegetable: The Herb Farmacy

SBOH Action: Motion by L. Firth; seconded by Joanne Housianitis. Unanimously voted to renew license if appropriate inspection has taken place.
